The CMA Foundation’s Advancing Practice Excellence in Diabetes project is proud to announce the launch of its new and improved website, designed to provide users with the best and most up-to-date diabetes management tools and information.
The website offers information about the project and how to get involved as well as an array of patient and provider resources. A key resource feature is the Multicultural Patient Education Materials Database which allows patients and providers to search materials by topic, target audience and language.
Advancing Practice Excellence in Diabetes has three regional pilot projects in San Joaquin County, the Butte/Glenn area, and the Inland Empire. The focus of the project is to assist physicians in improving the quality of care given to diabetes patients, and aims to provide health care providers with resources to increase their clinical knowledge, communicate more effectively with patients, and broaden their understanding of the cultural health disparities associated with diabetes.
